This project is a systematic review looking at interventions in healthcare settings to reduce social isolation. The purpose of this research is to aggregate data on interventions that address social isolation in an ambulatory healthcare settings to gain insight into the key ingredients of successful interventions.

Impact

Social Isolation was identified as a priority that the St. Michael’s Hospital Academic Family Heath Team wished to address in their patient population. We anticipate that this systematic review, along with qualitative interviews with patients and providers, will inform an evidence-based pilot intervention that will reduce social isolation for patients at the St. Michael’s Hospital Academic Family Heath Team.
